Spravato (Esketamine) Online? Why It's Only Given In Person

Searching for Spravato online?

People often search for "Spravato online," an online Spravato prescription, or esketamine nasal spray to use at home. You can't get Spravato any of those ways. Under the Spravato REMS, certified pharmacies send Spravato only to certified healthcare settings, never to patients. Every dose is taken there under a healthcare provider's direct observation, followed by at least 2 hours of monitoring, and a telehealth visit can't replace that session. If a website offers to sell or ship esketamine to you, it isn't following these rules.

What you can do instead:

• If you live somewhere else, ask your psychiatric clinician about a certified Spravato treatment center near you.
• If you want care by video, ask about our at-home ketamine evaluation. Ketamine is a different medication from Spravato. It's prescribed off-label, it isn't FDA-approved for any psychiatric condition, and it isn't right for everyone. A licensed clinician decides whether it fits you after an evaluation.

We can't prescribe controlled substances, including ketamine, in Alabama, Arkansas, Hawaii, Louisiana, Maine, Massachusetts, Minnesota, Missouri, Oklahoma, Pennsylvania and West Virginia, and we don't offer ketamine in Texas. Federal telehealth rules for controlled substances are temporary through Dec 31, 2026 and may change.

Spravato is for adults; it isn't approved for children or teens. If you'd rather not add a medication, TMS therapy in Las Vegas is a non-drug treatment for depression given at the same office.

Who Spravato is approved for

• Treatment-resistant depression in adults, used on its own or with an oral antidepressant. In the clinical trials, this meant major depression that hadn't improved enough after at least two different antidepressants, each taken at an adequate dose for an adequate time.
• Depressive symptoms in adults with major depressive disorder who have acute suicidal thoughts or actions, used with an oral antidepressant.

Spravato isn't recommended during pregnancy because it may harm an unborn baby, and breastfeeding isn't recommended during treatment. Tell your clinician if you're pregnant or planning a pregnancy.

Spravato hasn't been shown to prevent suicide or reduce suicidal thoughts, and it doesn't replace a hospital stay when one is needed. If you're in crisis now, call or text 988 or go to the nearest emergency room. Whether Spravato is right for you is your clinician's decision after an evaluation.

What a Spravato treatment course looks like

For treatment-resistant depression, the label's schedule is:

• Weeks 1 to 4: two sessions a week.
• Weeks 5 to 8: one session a week.
• Week 9 on: one session every week or every 2 weeks, at the least frequent schedule that keeps you well.

Your clinician checks whether it's helping at the end of the first 4 weeks. For depressive symptoms with acute suicidal thoughts or actions, the course is two sessions a week for 4 weeks.

At each session your blood pressure is checked before and after the dose and your breathing is monitored. Don't eat for 2 hours or drink for 30 minutes beforehand, since nausea and vomiting can happen. You can't drive or operate machinery until the next day after a restful sleep, so plan a ride home.
