Planning a family when your mind and history matter

Dr. Sam Zand, DO
Dr. Sam Zand, DO

Founder & Chief Psychiatrist

Trying to conceive, navigating fertility treatments, or preparing for a first or next child can surface anxiety, grief, and old family patterns all at once. Patients ask whether they should stop psychiatric medications, how infertility is affecting their marriage, and whether the parenting they received will repeat in their own home. Those questions deserve thoughtful, non-judgmental answers.

Our approach is not to rush a pregnancy timeline or push a one-size-fits-all medication plan. We help you understand your options, coordinate with your OB-GYN or fertility team when appropriate, and build emotional skills for the family you are creating — including when the path is harder than expected.

We look at the whole family system, because planning rarely happens in isolation:

  • Infertility stress, treatment cycles, pregnancy loss, and decision fatigue
  • Psychiatric medication review and safe titration before and during pregnancy
  • Generational trauma, attachment patterns, and what you want to carry forward
  • Parenting preparation for first-time parents, blended families, and partners
  • Anxiety, depression, and mood symptoms that flare during family transitions
  • Partner and co-parent support so both adults feel included in the plan

What to know before you start — written by our clinical team, not a generic template.

When Building a Family Brings More Than Joy

For many people, the decision to grow a family opens doors to hope — and to every unresolved fear about being enough, being loved, or repeating painful history. Infertility can strain even strong partnerships. A prior pregnancy loss can make each new attempt feel terrifying. Existing anxiety or depression does not pause because you want a baby.

Family and pregnancy planning mental health care names those realities openly. You do not have to minimize grief to stay optimistic, or hide medication questions because you are afraid of being judged.

Psychiatric Medication and Pregnancy Planning

If you take antidepressants, mood stabilizers, ADHD medications, or other psychiatric treatments, stopping abruptly is not always the safest choice. Our psychiatric providers review your current regimen, psychiatric history, and pregnancy goals to discuss tapering, switching, or continuing treatment with informed risk-benefit conversations.

We coordinate with your OB-GYN, reproductive endocrinologist, or primary care clinician when helpful. No medication decision is guaranteed; every plan is individualized and revisited as your body and family plans change.

Therapy often runs alongside medication planning — especially when fear of relapse, prior postpartum episodes, or trauma history influences what feels safe.

Generational Trauma and Parenting With Intention

Many patients arrive at parenthood wanting something different from what they grew up with — but unsure how to break cycles of criticism, emotional distance, substance use, or chaos. Generational trauma counseling helps you map what you inherited, grieve what you did not receive, and choose what you want your children to experience.

Parenting preparation is not about perfection. It is about communication between partners, realistic expectations, nervous-system regulation under stress, and knowing when to ask for help. We support first-time parents, stepparents, and families adding another child.

Our Approach

Care That Honors Your Whole Story

Family-Aware

When appropriate, partners join sessions so fertility stress, parenting values, and communication patterns are addressed together — not in silos.

Safety-Focused

Psychiatric medication decisions are made with clear discussion of pregnancy and breastfeeding considerations — never rushed or one-size-fits-all.

Trauma-Informed

Generational trauma, loss, and attachment wounds are treated with respect — not as obstacles to overcome before you deserve support.

Evidence-Based

Care draws on perinatal psychiatry and therapy evidence, with transparent limits of telehealth and coordination with your medical team.

Your Family & Pregnancy Planning Intake

A structured conversation about fertility, medications, family history, and the support you need next.

Family & Fertility Intake

You share pregnancy goals, fertility history, current medications, mood symptoms, and family-of-origin patterns through secure forms and discussion.

Clinical Evaluation

Your provider assesses anxiety, depression, trauma history, and relational stress — including partner dynamics when relevant.

Coordinated Plan

Next steps may include therapy, medication review, partner sessions, or coordination with your OB-GYN or fertility specialist.
