Texas has the largest geographic footprint of any continental state and one of the worst psychiatrist-per-capita ratios — particularly outside the four major metros. The Texas Medical Board and HHSC have designated 218 of 254 counties as mental-health shortage areas. A patient in Big Bend, the Panhandle, or deep South Texas may be looking at a 200-mile drive to the nearest in-person psychiatrist.
